Submit Annual Financial Statements

CityWide requires current financial statements, such as tax returns and updated personal financial statements from each CityWide commercial loan and SBA 504 loan client on an annual basis. Please submit them by one of the following methods:

Change Account for Debit Payment

If you want to change the account that is debited for your monthly payment on any of CityWide’s loan programs, please submit a new ACH (Automated Clearing House) Authorization form and voided check to CityWide. The form must be received prior to the 13^th day of the month to take effect the following month.

Check My Balance

For CityWide commercial loans:

The monthly Payment Notice for CityWide commercial loans shows your loan’s current principal balance in the upper right-hand corner.

If you need further assistance with your loan balance please send your request to dburnett@citywidedev.com. Please be sure to include your name, business name, loan number and telephone number.

For SBA 504 loans:

You can view your loan balance at any time per your Amortization Schedule which was provided to you when your loan funded.  Your amortization schedule is a breakdown of your monthly loan payment for the life of your loan.  This document is provided to you in lieu of monthly statements.

Loan Interest Paid

If you would like to change the reporting of interest paid to the IRS on any commercial loan, please complete an IRS Form W-9 – Request for Taxpayer Identification Number and Certification form with the new name(s) and tax ID number and send it to jwhite@citywidedev.com.

If you need to request a copy of your Annual Statement of interest paid for CityWide commercial loans please send an email to dburnett@citywidedev.com.

If you need to request a copy of your Annual Statement for SBA 504 loans please send an email to jwhite@citywidedev.com.

Please be sure to include your name, the company name, loan number and property address on all requests.

Pay Off My Loan

For CityWide commercial loans:

Please contact Donna Burnett at 937-853-2549 to obtain a CityWide commercial loan payoff.

For SBA 504 loans:

Prepayments on SBA 504 loans are made on a monthly basis.  If you wish to pay off your outstanding balance please contact Janet White at 937-853-2541 to establish a payoff date.  Prepayments are only accepted on the 3^rd Thursday of each month and must be scheduled in advance.
